Responsibilities
Provides all case management services to the program consumers, implementation of an Individual Recovery Plan (IRP)
Facilitates supportive skill development, cognitive stimulation, and activities groups
Ensures service provision and documentation is consistent with external regulatory bodies (Medicaid, NJ Division of Mental Health Services)
Observes consumers self-administration of prescribed medication, as per agency procedures
Qualifications
BSW in social work, BS/BA in psychology, or a related field
Valid Driver’s License
Mental health/human services experience desired
