What are the responsibilities and job description for the Partial Care Specialist position at CBH Care?
Responsibilities:
- Provides all case management services to the program consumers, implementation of an Individual Recovery Plan (IRP)
- Facilitates supportive skill development, cognitive stimulation, and activities groups
- Ensures service provision and documentation is consistent with external regulatory bodies (Medicaid, NJ Division of Mental Health Services)
- Observes consumers self-administration of prescribed medication, as per agency procedures
Qualifications:
- BSW in social work, BS/BA in psychology, or a related field
- Valid New Jersey driver’s license
- Mental health/human services experience desired
Shift:
- Monday through Friday, 9am-5pm
Pay:
- $40,000 - $41,000 annual salary
