Key Responsibilities
- Deliver direct nursing care in accordance with individualized care plans, physician orders, and established clinical protocols
- Perform ongoing assessments of resident conditions, documenting findings accurately and reporting significant changes to the appropriate clinical personnel
- Administer medications, treatments, and therapeutic interventions in compliance with scope of practice, facility policy, and regulatory requirements
- Collaborate with interdisciplinary team members to ensure continuity of care and achievement of optimal clinical outcomes
- Provide education to residents and families regarding diagnoses, medications, and care plans to support informed decision-making
- Supervise and delegate tasks to CNAs and support staff, ensuring care is delivered safely, efficiently, and in accordance with established standards
- Maintain timely, accurate, and compliant documentation within the electronic medical record (EMR) system
- Participate in infection prevention, quality assurance, and performance improvement initiatives in alignment with KDADS and CMS standards
- Recognize and respond appropriately to changes in resident condition, including urgent and emergent situations, following established clinical protocols
- Ensure adherence to professional standards, confidentiality requirements, and all applicable federal and state regulations
- Graduate of an accredited practical nursing program with a current, active Licensed Practical Nurse (LPN) license in good standing in the State of Kansas
- Current Basic Life Support (BLS) or CPR certification
- Demonstrated ability to provide compassionate, resident-centered care across varying levels of acuity
- Proven ability to collaborate effectively within a multidisciplinary healthcare team
- Strong clinical judgment, critical thinking, and decision-making capabilities
- Effective communication skills with the ability to build trust with residents, families, and team members
- Commitment to quality care, regulatory compliance, and continuous improvement in resident outcomes
