Position Details:
- Main Office location: Westwood, MA
- Service Area: Quincy, Westwood, Brockton, Scituate, Attleboro areas.
- Job Type: Per Diem
Responsibilities:
- Conducting psychosocial assessments related to the patient's illness and environment and effectively communicating findings to the registered nurse.
- Performing social evaluations and developing appropriate intervention plans based on assessment findings.
- Maintaining accurate and up-to-date clinical records for all patients referred to social work.
- Providing essential information and referral services to patients, families, and caregivers regarding practical and environmental needs.
- Acting as a liaison between patients, families, caregivers, and community agencies.
- Building and maintaining collaborative relationships with other healthcare professionals to support comprehensive patient care.
- Working with public and private agencies to leverage available resources for the benefit of our patients and our organization.
- Participating in the development of comprehensive care plans and engaging in case conferences as required.
- Assisting physicians and the multidisciplinary team in understanding the social and emotional factors influencing health problems.
- Contributing to discharge planning and ensuring a smooth transition for patients and their families.
Qualifications:
- LICSW (Licensed Independent Clinical Social Worker) required.
- Sensitivity and understanding of end-of-life issues.
- Expertise in psychosocial assessment and treatment.
- A collaborative approach to working with a multidisciplinary team.
- At least one (1) year of experience in Hospice care preferred.
- Strong verbal and written communication skills, as well as good organizational skills.
- Maintain current BLS CPR certification.
- Possess a valid driver's license and own a reliable, insured vehicle.
Benefits & Compensation:
- Rates: $60 per visit mileage reimbursement.
