Job Title: Occupational Therapist (Home Health)

Job Summary
Partners in Home Care is seeking a dedicated and compassionate Occupational Therapist to join our home health team. This role is responsible for providing evaluation, treatment, and rehabilitation services to patients in their homes, while offering education and support to both patients and their families. The Occupational Therapist will coordinate care with an interdisciplinary team and ensure high standards of clinical practice, documentation, and compliance.
Essential Duties and Responsibilities
- Conduct comprehensive evaluations and develop individualized care plans tailored to patients' needs in the home setting.
- Coordinate care with physicians and other disciplines; request additional services as needed.
- Provide direct, skilled occupational therapy interventions as prescribed by the physician and outlined in the care plan.
- Utilize evidence-based practice, Medicare regulations, and Conditions of Participation in treatment planning and delivery.
- Adjust treatment plans and visit frequency based on ongoing assessment and patient response.
- Proficiently complete and score OASIS assessments, collaborating with the Quality team to ensure accuracy and compliance.
- Maintain working knowledge of insurance requirements and payer guidelines for home health services.
- Participate in multidisciplinary case conferences to contribute to holistic, patient-centered care plans.
- Document all services, communications, and any adverse events in the patient’s medical record using point-of-care tools.
- Support Quality Assurance and Performance Improvement (QAPI) initiatives.
- Meet or exceed productivity expectations and maintain accurate records of time and mileage in accordance with agency policy.
- Coordinate daily schedule with the scheduler and plan efficient routes to maximize continuity of care and reduce travel time.
- Supervise Occupational Therapy Assistants (OTAs) and conduct timely supervisory visits. Provide oversight to CNAs when OT is the sole active discipline.
- Manage patient care throughout the episode, including admissions, re-evaluations (30-day functional assessments), and discharges.
- Maintain current Montana OT licensure and fulfill continuing education requirements.
- Adhere to ethical standards as defined by the OT licensure board and agency policy.
- Participate in agency-sponsored trainings and continuing education opportunities.
- Perform other duties as assigned by supervisor.
Minimum Qualifications
- Bachelor’s degree or higher from an accredited Occupational Therapy program.
- Current licensure to practice Occupational Therapy in the State of Montana.
- Valid driver’s license and reliable transportation with appropriate auto insurance.
- 2 years of occupational therapy experience preferred, but not required.
- Experience in home health or a similar field is a plus.
Schedule
Typical workdays are Monday through Friday. Schedule may vary depending on FTE status and patient care needs, as directed by the supervisor.
