- Looking for a short term contract PT 6-12 Weeks, PRN, PT, or FT - $75.00 to $90.00/HR (DOE)
Partners in Home Care is seeking a dedicated Physical Therapist to provide skilled evaluation and rehabilitation services to patients in their homes. This role includes direct care, care planning, patient/family education, and coordination with an interdisciplinary team to ensure high-quality, evidence-based outcomes.
Key Responsibilities
- Evaluate patients and develop individualized care plans in the home setting
- Provide direct, skilled physical therapy per plan of care and physician orders
- Adjust treatment frequency and duration based on patient progress
- Complete and score OASIS assessments; collaborate with the Quality team
- Maintain knowledge of Medicare regulations, Conditions of Participation, and payer requirements
- Perform physical therapy interventions, including wound care when appropriate
- Document care and communication using point-of-care systems
- Coordinate with schedulers, optimize travel routes, and communicate availability
- Participate in care conferences and agency quality improvement initiatives
- Supervise Physical Therapist Assistants and CNAs as required
- Plan and manage admissions, re-evaluations, and discharges
- Maintain licensure and continuing education standards
- Uphold ethical standards and participate in agency training sessions
- Bachelor’s degree or higher from an accredited Physical Therapy program
- Current license to practice in the State of Montana
- Valid driver’s license, auto insurance, and reliable transportation
- Access to high-speed internet
- 2 years of experience preferred, especially in home health or clinical rehab
