The Office LPN supports Home Health & Hospice by reviewing clinical documentation, assisting with chart audits, and providing overflow patient visits when needed. This role helps ensure accuracy, compliance, and continuity of care.
The Office LPN supports Home Health & Hospice by reviewing clinical documentation, assisting with chart audits, and providing overflow patient visits when needed. This role helps ensure accuracy, compliance, and continuity of care.
Key Responsibilities:

Review and audit Home Health & Hospice clinical notes for accuracy and compliance.

Assist with quality assurance tasks, documentation follow-up, and pre-survey readiness.

Provide direct patient care visits in the field during high census, staff shortages, or urgent needs.

Perform assessments, wound care, medication administration, and other LPN-level services.

Document visits accurately in the EMR.

Support office functions including care coordination, follow-up calls, and maintaining patient records.

Collaborate with the interdisciplinary team and assist with communication between field staff and leadership.
Qualifications:

Current LPN license in good standing.

Home health or hospice experience preferred.

Strong attention to detail and comfort reviewing documentation.

EMR experience preferred.

Valid driver’s license and reliable transportation for field visits.
