What are the responsibilities and job description for the Mental Health Program Coordinator position at Ozark Physical Medicine Center for Operations?
Program Coordinator needed for providing oversight and coordination of operations
Assist with Direct Support duties when coverage is needed in individuals homes
Duties include:
Coordinate and participate in the development of the Individualized Support Plan (ISP) and ensure outcomes are measurable and clear to staff
Ensure that staff are trained in the individual's written plan and in their responsibility for implementing the plan
Ensure accurately and timely documentation is completed for authorized services and supports
Supervise, evaluate and schedule support staff
Conduct record reviews to ensure records are current, completed and contain substantial documentation that services were provided as authorized
Conduct home visits to ensure premises are safe and clean, no need for repairs and furniture is adequate for storage and in good condition
Conduct face to face visits to observe the individual receiving support
Make visits to the ISL to monitor status of the individual and review required documentation
Ensure all staff have current trainings, including any training specific to the individual
Coordinate the provision of medical care to those served
Ensure medications and treatments are given as ordered by the physician
Review medical appointment summary and ensure recommended follow-up if any are completed
Review monthly health summary by the staff and community RN and ensure follow up recommendation are completed
Ensure money counts and controlled drugs are done at the beginning and end of shifts
Review event reports for completeness and ensure follow up if any are completed
Complete monthly summaries on individual's progress or lack of progression on goals and objectives in the ISP
Education:
- Bachelor's (relevant experience in the field of Developmental Disabilities can be substituted year for year for a degree)
Experience:
- 1 year with Bachelor's Degree or 5 years experience in specific field
License/Certification:
- CPR Certification (Preferred)
- Medication Aide (Preferred)
