We are seeking a compassionate and dependable PRN Nights & Weekend On-Call Hospice Clinician to provide high-quality, patient-centered care to individuals receiving hospice services. This role is responsible for responding to after-hours calls, providing in-home visits as needed, and supporting patients and families during critical moments.
Key Responsibilities
- Respond promptly to after-hours and weekend on-call requests
- Provide skilled nursing care and symptom management in patient homes or facilities
- Perform assessments and interventions to ensure patient comfort and safety
- Communicate effectively with patients, families, and interdisciplinary team members
- Document all patient interactions in a timely and accurate manner
- Provide emotional support and education to patients and caregivers
Qualifications
- Active and unrestricted RN license in the state of MO
- Minimum of 2 years of clinical experience (hospice or home health preferred)
- Strong assessment and critical thinking skills
- Ability to work independently and make sound clinical decisions
- Reliable transportation and valid driver’s license
- Current CPR certification
Preferred Qualifications
- Previous hospice or palliative care experience
- Experience with electronic medical records (EMR)
- Comfort with end-of-life care and family support
