Responsibilities:
The Case Manager, Level 1, provides supportive case management services to Adults and Young Adults with mental illness in the apartment treatment and supportive housing program. Monitors maintenance of sites and provides on site visits to scattered housing locations.

- High School Diploma or the equivalent and 2 years of related experience, or a Bachelor's degree and 1 year of related experience.
- Experience working with young adults is preferred.

- 8:00 AM – 4:30 PM (1 hour lunch break)

- 1 day/week,
- 11:00 AM – 7:00 PM (30-minute lunch break)
- 10:30 AM – 7:00 PM (1 hour lunch break)

Responsibilities:
The Case Manager, Level 1, provides supportive case management services to Adults and Young Adults with mental illness in the apartment treatment and supportive housing program. Monitors maintenance of sites and provides on site visits to scattered housing locations.
- Plan and evaluate service plans and monitor objectives. Write progress notes weekly.
- Reviews case management plans and prepares periodic reassessments.
- Perform site visits 3-5 times weekly with apartment treatment residents and 1-3 times weekly with supportive housing residents, dependent on client need and desire.
- Monitor maintenance of sites.
- Provide information regarding community resources and orient residents to the area.
- Assist in securing vocational assessment services, job training and education programs.
- Provide advocacy services to gain appropriate entitlements and services.
- Intervene in situations requiring immediate attention to ensure safety of residents.
- Provide mediation, counseling, behavioral intervention, and crisis management services.
- Consult and coordinate with clients’ family and providers as needed for effective service delivery.
- Train and assist residents in the activities of daily living including household maintenance.
- Notify program supervisor/manager of any medical or psychiatric emergency.
- Oversee medication maintenance and assist residents in the development of self-medication skills as needed.
- Attend team meetings and discharge meetings as necessary.
- Ensure household cleanliness is maintained to agency standards and assist as needed. Conduct fire drills at sites as needed.
- Any additional relevant tasks as assigned by management.
