Responsibilities
- Work as an onsite case manager and member of the team to support youth charged with gun-related offenses.
- Provide individualized case management for all participants.
- Make appropriate cross-system referrals i
- Life Skills: Harm reduction, parenting, healthy habits, financial literacy, and planning.
- Manage, build and create relationships with local agencies and service providers.
- Assist youth in navigating systems including providing court advocacy and transportation to medical appointments.
- Work with the court staff/resource coordinators, mentors and therapeutic counselor and assist them in providing case management services to their high-risk program participants.
- Coordinate case planning/ service delivery with assigned court staff, including Resource Coordinators, and make appropriate cross-system referrals.
- Engage with other community-based service providers as appropriate to ensure a wraparound approach.
- Be available to support the team in emergencies regarding incidents with staff or participants.
- Adhere to all documentation and database requirements.
- Bachelors Degree in related field with appropriate experience.
- Bi-lingual English-Spanish required.
- 2 years of experience in working with at risk populations.
- Lived experience required and connections with at-risk Hempstead youth a plus.
- Proficient computer skills with MS Office
- Valid and clean driver's license.
