Key Responsibilities
- Conduct comprehensive patient assessments and deliver skilled nursing care in home environments
- Develop and implement individualized care plans in collaboration with physicians and the interdisciplinary care team
- Educate patients and families on treatment plans, medications, and post-care instructions
- Monitor patient progress, evaluate outcomes, and make necessary care adjustments
- Ensure accurate and timely documentation in accordance with clinical and regulatory standards
- Coordinate with healthcare professionals to support continuity and quality of care
- Monday through Friday, 8:00am–5:00pm
- One weekend per month (on-call)
- Two holidays per year
- Flexible scheduling with minimal weekend work
- This is a Pay Per Visit position
- Local travel required within the Traverse City area (travel reimbursement provided)
Required
- Associate degree or higher in Nursing
- Active Registered Nurse (RN) license in Michigan
- Minimum of 1 year of professional RN experience (home health preferred)
- Current BLS or ACLS certification through the American Heart Association
- Valid driver’s license, reliable transportation, and current auto insurance
- Previous home health or community-based nursing experience
- Excellent assessment, communication, and organizational skills
