Key Responsibilities
- Deliver skilled nursing care and conduct thorough patient assessments in the home environment
- Develop and implement individualized care plans in coordination with physicians and the interdisciplinary team
- Administer medications, treatments, and interventions per physician orders
- Educate patients and family members on care plans, medications, and ongoing health management
- Monitor patient progress, documenting all care and changes accurately and promptly
- Communicate effectively with the healthcare team to ensure seamless continuity of care
- Monday through Friday, 8:00am–5:00pm
- On-call weekend rotation every 4–8 weeks (Saturday and Sunday, 8:00am–5:00pm for admissions as needed)
- Two weekdays off following on-call weekends
- Flexible scheduling with minimal weekend requirements
- This is a Pay Per Visit position
- Local travel required within the Lancaster, PA area (travel reimbursement provided)
Required
- Associate degree or higher in Nursing
- Active Registered Nurse (RN) license in Pennsylvania
- Minimum of 1 year of RN experience (home health preferred)
- Current BLS or ACLS certification through the American Heart Association (assistance provided if needed)
- Valid driver’s license, reliable transportation, and current auto insurance
- Prior home health or community-based nursing experience
- Strong clinical judgment, organization, and communication skills
