Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patients’ health by taking vital signs such as blood pressure, pulse, temperature, and respiration; report any concerns promptly
- Support medication administration following care plans and HIPAA guidelines to ensure patient confidentiality and safety
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Maintain a clean and safe environment by performing light cleaning duties and ensuring proper sanitation practices
- Observe and document patient conditions, behaviors, and progress to inform care plans and communicate updates to healthcare professionals
- Utilize equipment such as Hoyer lifts or other assistive devices to safely transfer residents with mobility challenges
Experience
- Prior experience working in assisted living, nursing homes, or home health care settings preferred; group home experience is a plus
- Knowledge of patient observation techniques and ability to recognize signs of health deterioration or distress
- Familiarity with dementia care, Alzheimer's care, memory care, autism support, or working with individuals with developmental disabilities highly desirable
- Certification in CPR and First Aid required; additional certifications in hospice care or home & community care are advantageous
- Experience with medication administration, care planning, and working within HIPAA regulations is highly valued
- Ability to lift heavy objects safely and assist residents using equipment like Hoyer lifts or other mobility aids
- Strong communication skills for collaborating with healthcare teams, social workers, residents’ families, and other caregivers
