Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ health by taking vital signs and observing for any changes in condition or behavior
- Administer medications accurately following care plans and HIPAA privacy regulations
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking
- Support clients with memory care needs such as Alzheimer’s or dementia by implementing specialized techniques and routines
- Use equipment like Hoyer lifts or resident’s
to ensure safe transfers and mobility assistance - Maintain detailed documentation of patient care activities, observations, and any incidents in compliance with healthcare standards
- Perform light housekeeping duties such as cleaning client living areas to promote a safe and hygienic environment
- Collaborate with healthcare professionals to develop and update personalized care plans
- Provide emotional support and companionship to foster a positive, engaging atmosphere
Skills
- Proven experience in home & community care, assisted living, nursing homes, or long-term care settings
- Knowledge of patient observation techniques, vital signs monitoring, and medication administration
- Strong understanding of dementia, Alzheimer’s, autism spectrum disorder, developmental disabilities, hospice care, and memory care practices
- Ability to perform heavy lifting safely using appropriate equipment like Hoyer lifts or other assistive devices
- Excellent communication skills for working with clients, families, and healthcare teams
- Familiarity with HIPAA regulations to ensure confidentiality and privacy of patient information
- CPR and First Aid certification to respond effectively during emergencies
- Experience working with individuals with disabilities or special needs in group home or residential settings is preferred
- Ability to follow detailed care plans accurately while maintaining flexibility for individual needs
