Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, noting vital signs and changes in condition to ensure safety and well-being
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking
- Administer medications accurately following care plans while maintaining strict adherence to HIPAA regulations for patient privacy
- Support clients with mobility using equipment such as Hoyer lifts or transfer belts, ensuring proper safety protocols are followed
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces
- Document care activities meticulously, updating care plans regularly and communicating effectively with healthcare teams or family members
Requirements
- Proven experience in home & community care, assisted living, nursing homes, or group homes is highly preferred
- Knowledge of patient observation techniques, vital signs monitoring, medication administration, and emergency procedures including CPR and first aid
- Experience working with individuals with developmental disabilities, autism, Alzheimer’s disease, dementia, or memory care needs
- Ability to lift heavy objects safely and operate mobility aids like Hoyer lifts or wheelchairs when necessary
- Strong understanding of HIPAA regulations to protect patient confidentiality at all times
- Excellent communication skills to collaborate with healthcare professionals, families, and clients effectively
- Valid certifications in CPR and first aid; additional training in hospice or long-term care is a plus
