Role and responsibilities
We’re growing and looking for two exceptional Nurse Practitioners to join our team:
- General Nurse Practitioner (NP) – Focused on mobile primary care, wound care, TCM, and palliative care
- Certified Behavioral Health Geri-Psych NP – Supporting patients with dementia, Alzheimer’s, and behavioral health needs
- Provide in-home, facility, and personal care home visits
- Collaborate with our interdisciplinary team to deliver holistic care
- Participate in CMS GUIDE and behavioral health initiatives
- Build meaningful relationships with patients, families, and community partners
- Document care using our EMR system (eClinicalWorks)
- Active NP license in Texas
- Certification in Behavioral Health or Geriatric Psychiatry (for Geri-Psych NP role)
- Passion for mobile care and serving vulnerable populations
- Excellent communication and teamwork skills
- Experience with dementia, wound care, or transitional care is a plus
