Key Responsibilities
- Conduct comprehensive geriatric assessments, including physical, cognitive, and psychosocial health.
- Diagnose, manage, and treat acute and chronic conditions common in older adults (e.g., dementia, polypharmacy, falls).
- Develop and implement individualized care plans, focusing on prevention, wellness, and functional independence.
- Prescribe medications and therapies, making referrals, managing complex medication regimens.
- Collaborate with physicians, social workers, therapists, and families.
- Educate patients and families on health management and care transitions.
- Advocate for the unique needs of the elderly within the healthcare system.
- Documenting visits, charting, clinical notes, and completing all other documentation requested after each patient visit.
Qualifications & Experience
- Master's or Doctoral degree in Nursing (MSN/DNP) with AGPCNP or Geri NP certification.
- Current RN and APRN licenses in Texas.
- DEA license, BLS/ACLS certification.
- Experience in primary care, acute care, or long-term care with older adults.
- Strong clinical skills in geriatric assessment, pharmacology, and chronic disease management.
- Excellent communication, leadership, and teamwork skills.
