💼 Your Role:

• Provide direct patient care and case management in the home
• Conduct patient admissions and develop individualized care plans
• Obtain medical history and perform physical exams (vitals, mental status, diet, etc.)
• Deliver skilled nursing care in compliance with HIPAA
• Evaluate effectiveness of nursing services and coordinate total patient care
• Determine need for LPNs, Home Health Aides, or Personal Care Attendants
• Coordinate paramedical services (PT, medical social services, etc.)
• Supervise LPNs, HHAs, and Personal Care Attendants as needed
• Submit patient notes and documentation timely

🎓 Qualifications:

• Licensed RN
• Must speak Portuguese
• OASIS and WellSky (Kinnser) experience preferred (training provided)
• Med-surg and wound care experience preferred
• Physical exam within 6 months prior to first patient contact
• Current TB test (within 12 months) & CPR certification
• Valid driver’s license, auto insurance, and clean driving record
• Ability to attend 1–3 day orientation at Taunton office
