Your Role:
- Collect patient medical histories and assess physical health (vitals, mental status, diet, etc.)
- Educate patients and families on nursing services and care plans
- Provide skilled nursing care in accordance with care plans and HIPAA
- Evaluate effectiveness of nursing interventions and adjust care plans as needed
- Submit accurate and timely patient notes/documentation
- Coordinate additional services as needed (LPN, HHA, Personal Care Attendant)
- Arrange referrals for services like PT or medical social work
- Supervise and support LPNs, HHAs, and Personal Care Attendants
Qualifications:
- Valid RN license
- OASIS and WellSky (Kinser) experience preferred (training provided)
- Med-surg and wound care experience a plus
- TB test within last 12 months & CPR certification
- Physical exam within last 12 months
- Valid driver’s license
- Ability to attend orientation at Taunton office
