Your Role:
- Provide direct patient care and case management in the home
- Conduct patient admissions and develop individualized care plans
- Obtain medical history and perform brief physical exams (vitals, mental status, diet, etc.)
- Deliver skilled nursing care in compliance with HIPAA
- Evaluate effectiveness of nursing services and coordinate total patient care
- Submit patient notes and documentation in a timely manner
Qualifications:
- Licensed LPN in Massachusetts
- Physical exam within 12 months prior to first patient contact
- TB test within 12 months & CPR certification
- Valid driver’s license
- Ability to attend 2-day orientation at Taunton office
