💼 Your Role:
- Provide direct patient care and case management in the home
- Conduct patient admissions and develop individualized care plans
- Obtain medical history and perform physical exams (vitals, mental status, diet, etc.)
- Deliver skilled nursing care in compliance with HIPAA
- Evaluate effectiveness of nursing services and coordinate total patient care
- Determine need for LPNs, Home Health Aides, or Personal Care Attendants
- Coordinate paramedical services (PT, medical social services, etc.)
- Supervise LPNs, HHAs, and Personal Care Attendants as needed
- Submit patient notes and documentation timely
🎓 Qualifications:
- Licensed RN
- Must speak Portuguese
- OASIS and WellSky (Kinnser) experience preferred (training provided)
- Med-surg and wound care experience preferred
- Physical exam within 6 months prior to first patient contact
- Current TB test (within 12 months) & CPR certification
- Valid driver’s license, auto insurance, and clean driving record
- Ability to attend 1–3 day orientation at Taunton office
