Job Summary:
We are seeking a compassionate and detail-oriented Virtual Care Coordinator to support telehealth services within a skilled nursing environment. This role involves coordinating virtual care operations, supporting patients and providers, and ensuring efficient, secure delivery of telehealth services.
Key Responsibilities:
- Perform quality assurance and peer reviews for telehealth and virtual observation programs
- Evaluate program performance and recommend improvements
- Meet with senior leadership to review metrics, feedback, and workflow enhancements
- Lead case reviews with hospital partners and monitor outcomes
- Triage and assign cases for virtual monitoring and observation
- Collaborate on telehealth policies, workflows, and implementation strategies
- Maintain and manage operational databases, including performance metrics, response times, and quality indicators
Qualifications:
- High School Diploma required
- Minimum 2 years of telehealth experience required
- Experience in direct patient care preferred
- Strong computer skills and familiarity with EMR systems
- Excellent organizational and communication skills
- Ability to multitask with strong attention to detail
