Type: Full-Time/ Direct Hire
Compensation & Benefits:
Minimum: $34.25/hr
Midpoint: $42.82/hr (approximately 10 years of experience)
Maximum: $47.10/hr (15 years of experience)
Sign-On Bonus: $10,000 (Paid in 5 installments over a 2-year commitment)
Relocation Assistance: Available on a case-by-case basis
Shift & Schedule: Day Shift (8:00 AM – 4:30 PM) | Rotating Weekends Required
Position Overview:
Navitas Healthcare, LLC is seeking RN - Case Manager for an exciting Travel or Local job in Fort Walton Beach, FL
Key Responsibilities:
• Guide patients and families through rehabilitation and care program education, including treatment plans, discharge criteria, insurance benefits, and patient rights
• Coordinate patient, family, and caregiver education to support care planning and recovery goals
• Develop and implement individualized treatment and discharge plans
• Complete psychosocial assessments and discharge planning documentation
• Collaborate with physicians, nursing staff, and interdisciplinary teams for care coordination
• Advocate for safe and appropriate discharge planning across the continuum of care
• Maintain communication with patients, families, and healthcare teams regarding care progression
• Promote patient-centered care and organizational values
Qualifications:
Education & Licensure:
• Registered Nurse (RN) license required
• Associate Degree or Nursing Certificate required
Experience:
• Minimum 2 years of recent acute care experience within the last 3 years required
• Preferred experience in ICU, CCU, Med-Surg, LTAC, or Hospital Case Management
Preferred Skills:
• Strong critical thinking and organizational skills
• Effective communication and patient advocacy
• Ability to collaborate in interdisciplinary teams
• Comfort with discharge planning and care coordination
• Adaptability and problem-solving mindset
