- $34.25/hr (Minimum)
- $42.82/hr (Midpoint – ~10 years experience)
- $47.10/hr (Maximum – 15 years experience)

Position Overview:
Navitas Healthcare, LLC is seeking RN Case Manager for an exciting Travel or Local job in Fort Walton Beach, FL.
Key Responsibilities
- Guide patients and families through rehabilitation and care program orientation
- Educate patients and caregivers on treatment plans, insurance benefits, discharge criteria, and patient rights
- Develop and implement individualized patient-centered care plans
- Complete psychosocial assessments and discharge planning evaluations
- Coordinate family conferences and interdisciplinary treatment discussions
- Collaborate with physicians, nurses, and care teams
- Advocate for safe discharge planning and continuity of care
- Maintain accurate and timely clinical documentation
- Promote compassionate and high-quality patient care
Qualifications
- Active Registered Nurse (RN) license required
- Associate Degree or Nursing Certificate required
- Minimum 2 years acute care experience within last 3 years required
- Preferred backgrounds: ICU, CCU, Med-Surg, LTAC, or Case Management
- Strong communication, critical thinking, and patient advocacy skills required
