🕒 Full-Time | Days | Mon-Fri 9:00 AM - 5:00 PM

📅 Weekend Rotation: 1 weekend per month

🔹 What You'll Do:

✅ Conduct comprehensive patient assessments upon admission

✅ Coordinate multidisciplinary care and discharge planning

✅ Reduce readmissions and improve patient outcomes

✅ Facilitate transitions of care and post-acute arrangements

✅ Collaborate with physicians, caregivers, payers, and healthcare teams

✅ Perform utilization review activities and payer communications

✅ Support safe discharge planning for diverse patient populations

🎓 Qualification

✔ Active NYS Registered Nurse License

✔ BSN Required

✔ Minimum 3 years of clinical nursing experience (ED, Critical Care, or Med/Surg preferred)

✔ Case Management experience preferred

✔ Knowledge of healthcare regulations, payer systems, and discharge planning

✔ Strong communication, organizational, and computer skills
