Job Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care across diverse healthcare settings. The ideal candidate will possess extensive clinical experience, including working with EMR/EHR systems, administering injections, and managing patient assessments. This role involves delivering compassionate nursing services in inpatient, outpatient, long-term care, and community environments, supporting patients with a wide range of medical needs such as geriatrics, pediatrics, trauma, and chronic conditions. The LPN will collaborate closely with multidisciplinary teams to ensure optimal patient outcomes while adhering to HIPAA regulations and safety standards.
Responsibilities
- Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations.
- Administer medications, injections, IV infusions, and perform venipuncture with sterile technique.
- Manage airway maintenance, trach, ventilator support, catheterization, and tube feeding procedures.
- Document all patient interactions accurately using EMR/EHR systems such as PCC
- Assist with diagnostic evaluations including laboratory specimen collection and processing, medical imaging coordination, and sonography.
- Provide specialized care for patients with complex needs such as Alzheimer's disease, dementia, traumatic brain injury recovery.
- Support discharge planning and case management activities for post-acute and home care patients.
- Educate patients and families on health coaching topics including chronic disease management and nutrition.
- Ensure infection control practices are followed rigorously to prevent healthcare-associated infections.
- Participate in team meetings to coordinate care plans for patients with diverse conditions such as infectious diseases, pediatric emergencies, or hospice needs.
Experience
- Proven experience working in Long term care. Prior roles in long-term care facilities including assisted living or nursing homes with dementia or memory care specialization.
- Experience managing complex cases involving pain management, ventilator support, dialysis, or post-surgical care
- Proficiency in using EMR/EHR systems like PCC for comprehensive medical documentation.
- Knowledge of infection control protocols and aseptic techniques, understanding and implementing EBP for all appropriate residents.
- Ability to work effectively with diverse patients.
