Job Purpose: To provide medical services as regulated by the State of Illinois Practical Nurse License and support necessary to sustain/maintain an individual’s physical health in the least restrictive environment possible.
Job Duties:
- Provide ongoing nursing care of an individual’s physical needs through direct and preventative medical methods
- Complete in-house nursing functions, such as monitoring weights, blood pressures, nutrition and providing medications to individuals and other vitals as ordered by physicians meeting regulatory standards
- Provide CPR/First Aid and emergency medical attention to individuals receiving services from the agency
- Complete required GERs with appropriate QIDPs being notified
- Assist individuals with medications as ordered
- Monitor hospital admissions and discharges as needed, including visiting them in an in-patient setting
- Assist in preparing appointment paperwork
- Educate individuals on paperwork/appointments and secure their signatures when necessary
- Assist in stocking and cleaning office when needed
- Secure individuals monthly weights
- Consult with agency QIDP’s regarding medical matters/appointments concerning individual caseloads
- Make arrangements for medical equipment for individuals when indicated
- Maintain in-patient emergency medication sheets and update electronic MARs with accurate and current medications as ordered by physicians
- Schedule and send notifications/consents to guardians for ancillary services including annual hearing tests, flu vaccines, and/or TB skin tests
- Communicate medical concerns and appointments with parents/guardians
- Visit CILA homes to make necessary changes to medications. Provide instructions to staff when necessary
- Attend individual Discovery meetings
- Assist with medical in-service training for staff and individuals including Basic Health and Safety
- Take on-call rotations with other nurses for 24 hour coverage and serve as a member of the crisis team to respond as needed
- Assist with the preparation for medical and psychiatric appointments which may include attending appointments, sending notification letters of appointments to parents/guardians, providing M.D. or psychiatrist with copies of all individuals information, obtaining consents for medications from parents/guardians, and turning in prescriptions to pharmacy for any new or changed medication
· Obtain 12 hours of Continuing Education each fiscal year as mandated by DHS
- Perform other medical duties as assigned

- Demonstrate or acquire a knowledge of human development, developmental disabilities, mental illness, medical terminology, therapeutic procedures, and community service
Qualifications:
- State of Illinois Licensed Practical Nurse license required
- Experience working with the intellectually disabled population preferred
- Valid driver’s license and proof of vehicle insurance required
Physical requirements:
· Ability to type, sit, stand, write and/or work on a computer for long periods of time
· Ability to access all agency locations to attend meetings, conduct interviews, and interact with employees
· Ability to independently lift and transfer up to 50 to 75 pounds to help individuals in and out of beds, wheelchairs, or bath tubs.
· Continuous movement, including frequent standing, walking, bending, stooping, kneeling and climbing stairs.
· Ability to see, hear, communicate, and safely perform the essential functions of the position, including driving agency vehicles, administering medications, recognizing environmental hazards, responding to emergencies, and monitoring the health and safety of individuals served, with or without accommodation.
