Are you passionate about supporting individuals with developmental disabilities and helping them thrive in community-based settings? The Moore Center is seeking a Program Manager – Enhanced Family Care to oversee individualized services for adults receiving supports through the Developmental Disabilities (DD) and Acquired Brain Disorder (ABD) Waiver programs.

In this role, you'll partner with providers, families, and interdisciplinary teams to ensure individuals receive safe, high-quality, person-centered services in Enhanced Family Care homes. You'll also play a key role in maintaining regulatory compliance, supporting providers, and advocating for the health, safety, and success of the individuals we serve.


Essential Responsibilities

  • Recruit, hire, train, supervise, and support Direct Support Professionals assigned to your caseload in accordance with agency policies and state regulations.
  • Monitor and oversee services provided by Enhanced Family Care providers to ensure quality, safety, and person-centered supports.
  • Ensure compliance with all applicable regulations, including He-M 1001, He-M 507, agency policies, and contractual requirements.
  • Report suspected abuse, neglect, or exploitation to designated agency leadership and Adult Protective Services in accordance with state regulations.
  • Partner with providers to identify, address, and communicate medical, behavioral, and quality assurance concerns in a timely manner.
  • Review documentation and records to ensure regulatory compliance and accuracy.
  • Conduct home visits to monitor provider residences for health, safety, and fire code compliance.
  • Prepare certified homes for state inspections and collaborate with the Manager of Certification on corrective action plans when needed.
  • Develop individualized goals and objectives in collaboration with the individual, family, providers, and interdisciplinary team.
  • Attend Service Agreement meetings, planning meetings, and other meetings related to individual services and placements.
  • Coordinate communication among providers, families, nursing staff, and other members of the individual's support team.
  • Implement services based on each individual's approved budget, residential proposal, and Annual Service Agreement.
  • Work closely with the Nursing Department to ensure timely follow-up on medical needs and quality assurance concerns.
  • Share agency updates, policies, and important information with providers.
  • Participate in interviewing, evaluating, and selecting prospective Enhanced Family Care providers.
  • Coordinate all aspects of client placements and transitions, including identifying providers, completing required documentation, facilitating moves, and monitoring successful placements.
  • Provide crisis intervention, emergency support, and problem-solving assistance for providers as needed.
  • Ensure providers complete all required initial and annual training requirements.
  • Monitor provider compliance with annual contracts and agency expectations.
  • Participate in an on-call rotation as assigned.
  • Maintain regular office attendance, collaborate effectively with team members, and perform other duties as assigned.

Qualifications

Education & Experience

  • Bachelor's degree preferred.
  • Minimum of two years of direct human services or related experience required.
  • Experience working with individuals with developmental disabilities or acquired brain disorders is strongly preferred.
  • Proficiency with Microsoft Office Suite and database systems.
  • Valid driver's license, reliable transportation, and automobile insurance that meets agency requirements. Use of a personal vehicle to transport individuals is required.

  • Represent The Moore Center, the Residential Department, and the Area Agency with professionalism and integrity.

All staff must adhere to the NH state law requiring mandated reporting of suspected abuse, neglect, or exploitation. Additionally, staff are required to maintain criminal, motor vehicle and Bureau of Elderly and Adult Services record checks that meet agency standards. Staff are also required to travel and transport clients when applicable. This includes having access to a reliable vehicle, a valid driver’s license and maintaining personal auto insurance of $100,000/300,000.
