Position Summary

The Service Coordinator is responsible for coordinating a diverse caseload of individuals with intellectual and developmental disabilities and/or brain injury. Serving as a primary point of contact for individuals and their families, this role coordinates and monitors services to ensure they are appropriate, effective, and aligned with each individual’s needs, preferences, and goals.

The Service Coordinator builds collaborative relationships with individuals, families, providers, and community partners to promote choice, independence, and meaningful community participation. Through ongoing assessment and service monitoring, this position helps ensure each individual has the support needed to live a self-directed, fulfilling, and enriched life.


Pay: $23/hr.

M-F-Full time

Requirements:


As a Service Coordinator, you’ll play an important role in helping individuals with intellectual and developmental disabilities and/or brain injury live full, meaningful, and self-directed lives. You’ll serve as an advocate and central point of contact, coordinating services and working closely with individuals, families, providers, and members of the support team.


Key Responsibilities
  • Facilitate Person-Centered Planning and develop individualized Service Agreements that reflect each person’s preferences, strengths, needs, and goals.
  • Monitor and evaluate individual outcomes on a monthly, quarterly, and annual basis, including conducting home visits and satisfaction surveys.
  • Promote and monitor the health, safety, and well-being of individuals receiving services.
  • Conduct and review assessments to identify individual needs and determine appropriate supports.
  • Develop meaningful goals based on individual preferences, assessments, needs, and desired outcomes.
  • Regularly review services to ensure they remain appropriate, effective, and responsive to changing needs.
  • Monitor services and supports to ensure they are provided in appropriate settings and are achieving positive outcomes.
  • Advocate for individuals by requesting revisions or adjustments to services when needed.
  • Review and understand individual budgets as part of ongoing service planning and coordination.
  • Maintain knowledge of He-M 503 regulations and other applicable state and agency requirements.
  • Build strong, collaborative relationships with individuals, families, service providers, and interdisciplinary team members.
  • Manage an active caseload independently while maintaining strong organization, communication, and timely follow-through.
  • Use Microsoft Office and agency database systems to maintain accurate and timely documentation.
What We’re Looking For
  • Bachelor’s degree preferred. Consideration may be given to candidates with an associate’s degree and at least two years of relevant human services experience.
  • 1–2 years of human services experience preferred.
  • Strong organizational and time-management skills with the ability to prioritize multiple responsibilities.
  • Excellent written and verbal communication skills.
  • Strong customer service, advocacy, problem-solving, and interpersonal skills.
  • Ability to work independently while also contributing effectively as part of an interdisciplinary team.
  • Demonstrated ability to work collaboratively with individuals and colleagues at all levels of an organization.
  • Creative, innovative approach to problem-solving and service planning.
  • Ability to interact respectfully and effectively with people from diverse backgrounds, experiences, abilities, and perspectives.
  • Proficiency with Microsoft Office, including Word, Excel, and Outlook, and the ability to learn new software and database systems.

  • Ability to comply with all applicable state, agency, and department policies and procedures.
Driving Requirements

This position requires regular travel to support individuals in their homes and communities.

  • Valid driver’s license and access to a safe, reliable vehicle during scheduled working hours.
  • Ability to use a personal vehicle to transport individuals when required.
  • Automobile insurance meeting agency requirements, including $100,000/$300,000 Automobile Liability Insurance.
