This role is ideal for candidates with a background in case management, care coordination, or human services who are passionate about helping others navigate complex systems.
What You’ll Do
- Perform client intake interviews and assess needs based on program guidelines
- Assist with application processing and eligibility verification
- Maintain and manage client cases from intake through resolution
- Provide referrals to community partners and support services
- Track case progress and ensure timely follow-ups
- Accurately enter and maintain case documentation in CRM systems
- Support clients through challenging situations with empathy and professionalism
- Bilingual (English/Spanish or English/Creole) strongly preferred
- Bachelors Degree
- 2 years of experience in case management, social work, healthcare coordination, or nonprofit services
- Familiarity with public assistance programs (Medicaid, LIHEAP, EHEAP, etc.) is a plus
- Strong communication and problem-solving skills
- Ability to manage multiple cases while maintaining attention to detail
- Experience in high-volume client environments is helpful
