Social Services Assistant / Weekends

$22-$25/hour commensurate with experience

Job Summary:

Unity Health and Rehabilitation Center, a leading provider of rehabilitation and skilled nursing care, is seeking a compassionate and dedicated Social Services Assistant to join our team in Miami, Florida. As a Social Services Assistant, you will play a vital role in providing emotional support and social services to our patients, residents, and their families. Responsibilities:

• Provide emotional support and social services to patients, residents, and their families
• Conduct social histories and assessments to identify patients' social and emotional needs
• Develop and implement individualized care plans to meet patients' social and emotional needs
• Facilitate group activities and events to promote socialization and a sense of community
• Collaborate with interdisciplinary teams to ensure comprehensive care and support
• Maintain accurate and up-to-date records and reports
• Participate in ongoing education and training to stay current on best practices in social work and rehabilitation
  
  
  

Requirements:

• Education: High School Diploma, Associate degree or higher in social work, human services, psychology, gerontology, sociology, counseling, or a related field preferred. Completion of training on long-term care social service practice, resident rights, psychosocial assessment, and care planning is strongly preferred.
• Experience: Must have, as a minimum, one (1) year of experience in a healthcare or long-term care setting preferred.
