We are partnering with a respected healthcare and social services organization seeking an Intake Specialist to support intake, screening, and triage efforts for individuals seeking community-based programs and services.

- Conduct intake assessments and screening for incoming callers and clients
- Evaluate immediate and ongoing needs to determine appropriate programs and services
- Assist with triage by prioritizing cases based on urgency and client circumstances
- Provide information on available community resources and support programs
- Maintain accurate client records, documentation, and referral information
- Support older adults and families in accessing services that promote independence and quality of life
- Perform additional administrative and departmental support duties as needed
- Bachelor’s degree REQUIRED
- Minimum 2 years of experience within social services, nonprofit organizations, healthcare support, human services, or case management environments
- Bilingual in English/Spanish preferred
- Strong communication, interpersonal, and customer service skills
- Comfortable working with older adults, families, and diverse populations
- Proficient in Microsoft Office Suite, including Word and Excel
- Strong typing and data entry skills (45 WPM preferred)
- Highly organized, compassionate, detail-oriented, and client-focused
