Intake Specialist

We are partnering with a respected healthcare and social services organization seeking an Intake Specialist to support intake, screening, and triage efforts for individuals seeking community-based programs and services.

• Conduct intake assessments and screening for incoming callers and clients
• Evaluate immediate and ongoing needs to determine appropriate programs and services
• Assist with triage by prioritizing cases based on urgency and client circumstances
• Provide information on available community resources and support programs
• Maintain accurate client records, documentation, and referral information
• Support older adults and families in accessing services that promote independence and quality of life
• Perform additional administrative and departmental support duties as needed

• Bachelor’s degree REQUIRED
• Minimum 2 years of experience within social services, nonprofit organizations, healthcare support, human services, or case management environments
• Bilingual in English/Spanish preferred
• Strong communication, interpersonal, and customer service skills
• Comfortable working with older adults, families, and diverse populations
• Proficient in Microsoft Office Suite, including Word and Excel
• Strong typing and data entry skills (45 WPM preferred)
• Highly organized, compassionate, detail-oriented, and client-focused
