Key Responsibilities Include
- Assessing patients’ functional abilities and home environments
- Developing and implementing individualized treatment plans
- Guiding interventions to improve mobility, strength, balance, and safety
- Educating and empowering patients and caregivers
- Collaborating with an interdisciplinary care team to achieve recovery goals
- Master’s or doctoral degree in Physical Therapy from an accredited program
- Current state license and at least 1 year of experience
- Reliable transportation and willingness to travel daily within the service area
- Preferred: Home health experience, OASIS certification, Medicare documentation experience
- Strong clinical assessment and treatment planning skills
- Excellent communication and problem-solving abilities
- Ability to work independently and adapt to varied home environments
- Interpersonal skills to interact effectively with patients, families, and healthcare teams
