- Perform patient transfers, ensuring safety and proper body mechanics.
- Provide compassionate care, respecting patient dignity and fostering engagement.
- Develop and modify treatment plans based on clinical findings and patient response.
- Educate patients, families, caregivers, and staff using effective teaching methods.
- Evaluate impairments, establish diagnoses, and document care according to policies.
- Screen patients for therapy needs and monitor support personnel performance.
- Complete objective testing, apply therapeutic techniques, and communicate progress with referring providers.
- Bachelor's Degree in Physical Therapy.
- Licensed Physical Therapist (PT) in Illinois.
- Basic Life Support (BLS) certification.
- Experience with acute stroke care, orthopedic and neurologic evaluations, and therapeutic modalities.
- Strong communication, documentation, and patient education skills.
