Role Overview

Key Responsibilities
The Medical Social Worker applies social work expertise and clinical knowledge to educate patients with kidney disease on treatment, health, and wellness options, aiming to improve their quality of life. They support patients' social and emotional well-being through psychosocial assessments, care planning, and service management to help them cope with chronic kidney disease and dialysis treatments.
Key Responsibilities
- Conduct psychosocial assessments and participate in care team reviews
- Provide management of services to support patient coping strategies
- Educate and empower patients regarding treatment choices
- Master’s Degree in Social Work from an accredited institution
- MSW required; LCSW/ACSW preferred (LCSW preferred in CA)
- Licensure or certification as required by the state (LMSW in some states)
- Preferable experience in health or human services; medical experience highly desirable
- At least one year of experience in behavioral or healthcare settings preferred
