- Serve as a primary contact for hospice referral sources, outside agencies, healthcare professionals, and field staff.
- Manage incoming calls, providing information to patients, families, and referral sources to facilitate access to services.
- Communicate arrangements for care initiation with patients, clients, and families.
- Conduct intake screenings, gather and analyze required information for preliminary admission decisions.
- Log referrals, prepare reports, respond to inquiries, and promote hospice services to prospective patients.
- Receive verbal orders from physicians where legally permitted to initiate care/services.
- Input customer data into management systems and provide telephonic and on-call support as needed.
- Graduate of an accredited nursing program.
- Current RN licensure in the applicable state.
- At least one year of RN experience; hospice or home health preferred.
- Strong communication, organizational, and interpersonal skills.
- Empathetic patient advocate and team-oriented professional.
- Ability to coordinate care and communicate effectively with diverse stakeholders.
- Commitment to providing compassionate, patient-centered care in a supportive environment.
