Description
Mental Health Association of Nassau County (MHANC) is a not-for-profit membership organization dedicated to improving mental health in our community through advocacy, education, program development, and the delivery of direct services. Our organization is looking for a motivated individual with a calling to help adults with mental illness by becoming a PROS Intake Coordinator.
Coordinate the intakes and screenings for new members to the PROS program. Assists Assistant Clinical Director with administrative oversight of program. Maintains caseload of individual counseling.
- Receive and process inquiries regarding referrals from other mental health providers.
- Collect required documentation from referral source.
- Engage new program members in individual sessions and groups to ensure smooth transition.
- Conduct initial intake and assessment of new members.
- Welcome and orient members on first day of program.
- Follow up with referral source if consumer misses intake/first day.
- Conduct outreach efforts to other mental health providers to increase referrals.
- Complete preadmission assessments for pending participants.
- Charting as required.
- Conduct PROS groups when required.
- Serves as a back up to PROS Advisors and Group workers/facilitators
- Maintains understanding of PROS regulations.
- Attend staff meetings and training as required.
- Covers Caseload as required
- Attend PROS Management meetings as required.
- Represent PROS at various external meetings and conferences.
- Assists Assistant Clinical Director in the supervision of the day-to-day operation of the PROS Clinic including scheduling, coordination of care of the members including immediate resolution of medication issues.
- Assists with the coordination and facilitation of staff meetings, case conferences, and staff training (in absence of Assistant Director).
- Assists Assistant Program Director with supervision of field interns.
- Assists Program Director with data management (OMH based systems)
- Responsible for coordinating and managing staff to include adequate coverage of program functions.
- Assists Program Director with the appropriate audit preparation and record maintenance.
- Accepts and follows through on other duties assigned.
- Assists with member de-escalation
Requirements
Education & Experience
- Masters Degree in Social Work, Mental Health Counseling or CRC required.
- LMSW preferred.
- Prior intake and assessment experience preferred.
Other Qualifications
- Must have clean NYS Drivers License.
- Must have basic knowledge of MS Word and basic internet.
- Must be willing to travel in the community as needed.
- Must be able to read and write the English language in a legible and understandable manner.
- Must possess the ability to deal tactfully with personnel, members, visitors and others.
- Must be knowledgeable of practices and procedures as well as laws, regulations and guidelines that pertain to community mental health programs.
- Must have patience, tact, cheerful disposition and enthusiasm as well as the willingness to handle situations.
- Must be willing to seek out new methods and principles and be willing to incorporate them into existing practices.
