Key Responsibilities:
- Coordinate and track incoming referrals from initial contact through completion
- Communicate proactively with referral sources, patients, and internal teams to ensure smooth transitions of care
- Provide support with prior authorizations, data entry, and billing as needed
- Ensure all necessary documentation is accurate and submitted in a timely manner
- Remain available for patient or referral communication outside of standard business hours when needed
Qualifications:
- LPN or Social Worker (strongly preferred)
- Experience in intake coordination, case management, or a similar healthcare role
- Familiarity with healthcare billing, prior authorizations, and insurance verification
- Excellent organizational and communication skills
- Tech-savvy and comfortable using EMR and data entry systems
