Bilingual Medical Social Worker

Salary: $53,000–$95,000/year, based on years of social work experience

Schedule: Monday–Friday | 8:00 AM–5:00 PM

We are seeking a compassionate and experienced Bilingual Medical Social Worker to support high-risk patients with complex medical, psychosocial, and financial needs. This role combines clinical social work, complex case management, care coordination, and patient advocacy.

The Medical Social Worker will work closely with patients, families, caregivers, healthcare providers, case managers, and community organizations to develop individualized care plans and connect patients with resources that improve access to care and overall quality of life.

Key Responsibilities

• Provide comprehensive care coordination and complex case management to an assigned patient caseload.
• Conduct clinical social work assessments and develop individualized treatment and care plans.
• Address the emotional, social, financial, and practical challenges associated with illness, disability, and aging.
• Connect patients and families with community resources, government programs, financial assistance, and social services.
• Assist patients with Medicare, Medicaid, SNAP, LIS, PAP, foundation applications, and other available assistance programs.
• Coordinate closed-loop referrals to community organizations and governmental agencies.
• Assist patients and families with advance directives, long-term planning, supportive care, and end-of-life decisions.
• Provide intervention and support in complex situations involving elder abuse/neglect, domestic violence, guardianship, mental health placement, and sexual assault.
• Collaborate with clinical and case management teams on transitions of care and referrals to post-acute providers.
• Advocate for patients and families by helping them understand healthcare benefits, applicable regulations, and available services.
• Support patient empowerment and independence in making healthcare decisions.
• Provide services across various settings, including in-home, clinic, hospital, skilled nursing facility, group home, and ancillary settings.
• Conduct in-person, telephone, and text-based patient engagement as appropriate.
• Accurately and timely document assessments, interventions, care plans, and patient interactions in the medical record.
• Maintain working knowledge of Medicare, Medicaid, long-term care programs, and other government/community resources.
• Participate in case management metrics and quality improvement initiatives.
• Provide professional education to staff and community partners.
• Facilitate company-sponsored patient or community support groups.

Required Qualifications

• Bilingual in English and Spanish.
• Graduate of an accredited Master of Social Work (MSW) program.
• Active Arizona license such as LMSW, LCSW, LPC, or LMFT.
• 3 years of social work experience with an applicable clinical license.

Alternative qualification: CHW certification with 6–8 years of social work experience, as applicable.

• Strong knowledge of government and community resources, including Medicare, Medicaid, long-term care, and social service programs.
• Experience assisting patients with LIS, PAP, SNAP, and foundation applications.
• Experience assessing and addressing Social Determinants of Health (SDOH).
• Working knowledge of ICD-10 and Z codes.
• Proficiency with EMR systems, Microsoft Office, computers, mobile devices, and other healthcare technology.
• Demonstrated ability to connect patients with appropriate community resources and ensure closed-loop referrals.
• Strong communication, advocacy, organization, and critical-thinking skills.

Preferred Qualifications

• Acute inpatient hospital social work experience.
• Experience working with senior/geriatric populations.
• Previous experience in complex case management or care coordination.
• Experience working with Medicare Advantage, Medicaid, or other government-sponsored healthcare programs.
