Job Description
The Community Health Worker serves as a bridge between individuals, healthcare providers, and community resources to improve access to care and promote positive health outcomes. This role provides outreach, health education, care coordination, resource navigation, and advocacy to help individuals overcome barriers to healthcare and social services. The Community Health Worker works closely with interdisciplinary teams to support individuals through personalized, community-based care.
Responsibilities/ Job Duties:
- Conduct community outreach to educate individuals and families about available healthcare programs, preventive services, and community resources.
- Promote health and wellness through one-on-one education, community engagement, and outreach initiatives.
- Assist individuals in navigating healthcare systems and accessing medical, behavioral health, and social service resources.
- Support enrollment into healthcare and community-based programs as appropriate.
- Manage an assigned caseload while coordinating referrals and ensuring timely follow-up.
- Connect clients with resources including healthcare, housing, transportation, food assistance, financial support, and other community services.
- Build trusting relationships by providing ongoing support, advocacy, and encouragement to individuals with complex health and social needs.
- Conduct home visits and community-based visits to assess client needs, identify barriers to care, and provide health education.
- Utilize motivational interviewing and other engagement techniques to encourage healthy behaviors and self-management.
- Collaborate with healthcare providers, case managers, social workers, and interdisciplinary teams to coordinate comprehensive care.
- Maintain accurate, timely, and confidential documentation of client interactions, assessments, referrals, and outcomes.
- Collect and report program data in accordance with organizational policies and regulatory requirements.
- Participate in team meetings, case conferences, and care planning discussions.
- Maintain knowledge of community resources, healthcare programs, and available support services to effectively assist clients.
Minimum Education and Experience Qualification Requirements:
- High school diploma or equivalent required.
- Associate's or Bachelor's degree in Social Work, Psychology, Public Health, Sociology, Human Services, or a related field preferred.
- Minimum of 2 years of experience in community health, outreach, case management, care coordination, or a related healthcare or social services setting.
- Experience conducting community outreach and connecting individuals to healthcare or social service programs preferred.
- Bilingual in English and Spanish.
- Community Health Worker (CHW) certification preferred but not required.
Schedule/ Shift: Mon-Fri: 8:00am - 05:00pm
Salary : $25 - $30
