Position Overview:

 

Living Water Clinic – Clínica Agua Viva is seeking an enthusiastic, dependable, hardworking, and community-focused Community Health Worker (CHW) / Promotora de Salud to help connect patients and community members with healthcare services, education, resources, and programs.

The Community Health Worker / Promotora serves as a trusted link between the clinic and the communities we serve. The ideal candidate enjoys working with people, is comfortable approaching and engaging community members, and is passionate about helping individuals and families improve their health and well-being.

 

The successful candidate must be fully bilingual in English and Spanish and willing to work outside of a traditional clinic environment, including community health fairs, outreach events, schools, churches, and other community locations. Some evening and weekend work is required.

 

Summary of Responsibilities:

  • Represent Living Water Clinic professionally and positively in the community.
  • Participate in and help coordinate community health fairs, outreach events, educational events, and clinic-sponsored activities.
  • Engage community members and educate them about available healthcare services and programs.
  • Conduct outreach and provide information in both English and Spanish.
  • Assist patients with understanding and accessing healthcare and community resources.
  • Help patients complete forms and applications when appropriate.
  • Assist patients with healthcare technology, including patient portals, electronic registration tools, and other clinic-related applications.
  • Provide basic health education using approved educational materials.
  • Promote preventive healthcare services, including screenings, well-child visits, women's health services, immunizations, chronic disease programs, and other clinic programs.
  • Help identify individuals who may benefit from available clinic programs and connect them with the appropriate department.
  • Provide information regarding community resources, including food assistance, transportation, insurance/coverage assistance, and other social services.
  • Conduct appropriate follow-up with patients and community members who have requested assistance.
  • Maintain accurate documentation of outreach activities, patient contacts, referrals, and services provided.
  • Assist with tracking participation and outcomes for community outreach programs.
  • Help prepare educational materials, outreach supplies, and event materials.
  • Assist with setting up and breaking down booths, tables, supplies, and equipment at community events.
  • Develop positive working relationships with schools, churches, community organizations, local agencies, and other community partners.
  • Work collaboratively with clinic providers, staff, leadership, and community partners.
  • Maintain patient privacy and confidentiality in accordance with HIPAA requirements.
  • Perform other community outreach and patient-support duties as assigned.
 

Education:

  • High School Diploma or equivalent preferred.
  • Community Health Worker/Promotora certification or applicable training preferred.
  • Willingness to complete company sponsored CHW/Promotora training and certification
 

Prior Experience:

  • Previous experience as a Community Health Worker or Promotora preferred.
  • Experience in healthcare, social services, community outreach, public health, or a related field preferred.
  • Familiarity with Medi-Cal populations and community resources preferred.
  • Knowledge of healthcare programs and resources available in Tulare County and surrounding communities preferred.
 

Skills:

  • Fully bilingual in English and Spanish required.
  • Ability to speak, read, and communicate effectively in both English and Spanish.
  • Excellent interpersonal and customer-service skills.
  • Friendly, approachable, and comfortable engaging with individuals, families, and groups.
  • Strong communication and listening skills.
  • Dependable, organized, and able to follow through with assigned responsibilities.
  • Positive attitude and ability to work effectively as part of a team.
  • Ability to communicate respectfully with individuals from diverse backgrounds.
  • Ability to work independently in community settings with appropriate direction.
  • Basic computer and technology skills.
  • Ability to learn and utilize patient portals and other healthcare technology.
  • Strong problem-solving and organizational skills.
  • Flexible and willing to assist wherever needed.
  • Ability to represent Living Water Clinic professionally at all times.
 

Schedule Requirements:

  • Full-Time or Part-Time, as assigned.
  • Must be willing to work occasional evenings and weekends based on community event schedules.
  • Evening and weekend activities may include health fairs, school and church events, community festivals, health education activities, and other outreach opportunities.
  • Must be willing to travel between clinic locations and community-based settings.
  • Advance notice will be provided whenever reasonably possible for evening and weekend assignments.
 

Physical Requirements:

  • Ability to travel between clinic locations and community settings.
  • Ability to stand and walk for extended periods during community events.
  • Ability to work indoors and outdoors depending on event requirements.
  • Ability to assist with setting up and breaking down outreach booths, tables, supplies, and equipment.
  • Ability to lift and carry reasonable event supplies and educational materials.
  • Reliable transportation.
