Registered Nurse (RN) - Assertive Community Treatment (ACT) Team - Community Mental Health

Overview

We are seeking a compassionate and community-focused Registered Nurse (RN) to join an interdisciplinary Assertive Community Treatment (ACT) Team supporting older adults with complex behavioral health and medical needs throughout the community. This is a highly impactful field-based role where you will work directly with vulnerable seniors in their homes and community settings to help them maintain stability, independence, and quality of life.

As the clinical anchor of the ACT team, you will collaborate closely with psychiatrists, social workers, care coordinators, and community providers to deliver integrated, person-centered care to individuals facing chronic mental illness and co-occurring medical conditions.

Key Responsibilities

Comprehensive Assessment & Care Planning

• Conduct integrated mental health and basic medical assessments for older adult clients
• Develop individualized, person-centered care plans in collaboration with the multidisciplinary ACT team
• Perform ongoing clinical evaluations to support continuity of care and treatment planning

Medication Management & Clinical Support

• Administer psychiatric medications and monitor medication adherence
• Assess and document medication side effects, changes in condition, and treatment response
• Collaborate with psychiatric providers and primary care physicians regarding medication and treatment needs

Community-Based Nursing Interventions

• Deliver evidence-based nursing interventions and rehabilitative services in home and community settings
• Participate in crisis intervention and de-escalation efforts to maintain client safety and stability
• Provide health education and wellness coaching to clients and families regarding chronic illnesses, medications, and healthy living strategies

Care Coordination & Team Collaboration

• Serve as a liaison between clients, healthcare providers, housing agencies, and community resources
• Lead medical discussions during daily ACT team meetings and case conferences
• Maintain accurate and timely clinical documentation within the Electronic Health Record (EHR) system

Qualifications

Required

• Active and unrestricted Registered Nurse (RN) license in the State of New York
• Strong understanding of psychiatric medications and behavioral health nursing principles
• Ability to perform rapid assessments and respond effectively during behavioral health crises
• Experience documenting within Electronic Health Record (EHR) systems
• Strong communication, organizational, and interpersonal skills
• Ability to work independently in community-based and field settings

Preferred

• Experience working with geriatric or older adult populations
• Psychiatric nursing, ACT/FACT team, mobile crisis, home care, or visiting nurse experience
• Knowledge of Medicare, Medicaid, housing resources, and NYC social services systems
• Bilingual proficiency, particularly Spanish-speaking
• CPI, Mandt, or other crisis de-escalation certification
• Experience managing complex behavioral health and chronic medical conditions
