Job Title: Physical Therapist Home Health
Location: Augusta, GA 30907
Shift : 8 am - 4:30 pm
Pay Range: $60/hr - $70/hr
Start Date: 10/26/2026
Job Type: Contract (16 weeks with possible extension)

Position Summary: We are seeking a compassionate and skilled Physical Therapist (PT) to provide high-quality home health therapy services to adult and geriatric patients in the community. The ideal candidate will have experience treating patients with geriatric, neurological, stroke, and orthopedic conditions and be comfortable working independently in a home-based environment.
Key Responsibilities:
- Perform comprehensive physical therapy evaluations in patients' homes.
- Assess functional mobility, strength, balance, gait, and physical limitations.
- Develop individualized treatment plans and establish short- and long-term goals.
- Provide skilled therapy interventions for geriatrics, stroke rehabilitation, and orthopedic conditions.
- Monitor vital signs and recognize changes requiring appropriate intervention.
- Identify environmental and safety barriers within the home.
- Recommend and educate patients on appropriate adaptive and assistive devices.
- Complete OASIS and Plan of Care documentation as required.
Requirements:
- Current GA Physical Therapist (PT) License or eligible Compact PT license.
- Minimum 1 year of PT experience required.
- AHA BLS certification required.
- Home health experience required
- COVID card (Medical/Religious exemptions are accepted)
- Experience treating adult and geriatric patients preferred.
- Strong clinical knowledge of orthopedic and neurological conditions.
- Excellent communication, documentation, organization, and time-management skills.
- Ability to work independently and travel between patient homes.
- Ability to safely perform patient transfers and utilize mobility/assistive equipment.
