Role Overview:
The RN Case Manager supports patient-centered care coordination for a commercial population, collaborating with internal teams to improve health outcomes.
Key Responsibilities:
Assess, plan, implement, monitor, and evaluate care options to optimize health status. Integrate evidence-based guidelines, develop care systems for early intervention, and assist with designing and evaluating patient-centered models. Collaborate with patients, families, providers, and the healthcare team to ensure coordinated services. Continuously monitor diagnostics, utilization patterns, and quality metrics, adjusting treatment plans as needed. Maintain documentation and serve as a resource for benefits and utilization management.
Qualifications & Skills:
Minimum two years RN experience; case management experience and certification preferred. Knowledge of healthcare laws, regulations, insurance policies, and healthcare IT systems is beneficial. Ability to work independently and in multidisciplinary teams. Active RN license is a plus. BSN is preferred.
