Registered Nurse - Case Manager

Role: RN Case Manager, responsible for patient-centered care coordination supporting our commercial population.

Key Duties: Assess, plan, implement, monitor, and evaluate care options to optimize health outcomes. Develop evidence-based, patient-centric treatment plans, promote early intervention, and foster a team approach involving patients, families, and providers. Manage utilization, ensure documentation, and adjust care plans based on data. Contribute to designing and evaluating care models to improve quality, efficiency, and clinical results.

Qualifications & Skills

• Minimum two years RN experience; case management experience and certification preferred.
• BSN preferred; knowledge of healthcare laws, insurance policies, healthcare IT systems, and claims processes advantageous.
• Ability to work independently and collaboratively; active RN license is a plus.
• Strong analytical, organizational, and communication skills are essential.
