POSITION SUMMARY:
The Supported Housing Case Manager is responsible for oversight and coordination of the Transitions Program. This position is focused on providing support, coordinating services, and assisting individual’s transition from Personal Care Homes into independent living. Case Managers are responsible for coordination with our statewide community partners and maintaining positive relationships in the personal care homes. Also Responsible for the oversight of program budgets to ensure accurate expenditures. Also works with Supported Housing Team to provide support and fill in when needed.
DUTIES AND RESPONSIBILITIES:
- Provide ongoing needs assessments for clients.
- Participate in developing treatment plans to ensure clarity, measurability, and applicability to the client.
- Provide ongoing assessment and encouragement in regard to clients Activities of Daily Living (ADL’s).
- Complete all documentation timely and accurately in the format approved by funding sources and regulatory statutes.
- Serve as Liaison with Statewide Community Partners
- Act as liaison between client and community; building relationships with families and cooperating entities to develop additional resources and coordinate services.
- Must have transportation and be willing to assist clients in meeting their needs by providing outings, transporting to scheduled appointments with various community advocacy agencies, etc.
- Attend all Meetings pertaining to Transitions Team
- Assist clients in managing and maintaining appointments and ensuring participation to prevent further hospitalization
- Monitoring and managing issued funding to support stability for clients in the community.
- Other duties as required/assigned by Team Leader and/or supervisors.
- Assist with all Supported Housing Programs when needed
- Requires travel within 10-County service area.
JOB REQUIREMENTS / EXPECTATIONS:
- This position is about 40% face to face with clients and 60% paperwork or researching.
- Transporting clients via personal vehicle or company vehicle
- Primary location is Corporate Office in Warren County in a clinical setting
- General knowledge and understanding of computers, and willingness to learn new systems.
QUALIFICATIONS:
- Bachelor’s degree in Psychology, social work, sociology or human services
- One year of experience working with SMI adults or one year of case management experience, post-graduation, or 6 months if individual possesses master’s degree in approved field.
- Valid drivers license, insurance and reliable transportation
