What are the responsibilities and job description for the Supported Housing Case Manager- HPP (53054) position at LIFESKILLS GROUP?
Provide Case Management services to individuals who are literally homeless, at risk for institutionalization or at risk for homelessness. This position involves direct services to clients and collaboration with community partners to assist the client in reducing barriers to achieving long term stable housing. Duties include, but are not limited to, assessing individuals’ strengths and needs; assisting with housing search and placement; assisting individuals with accessing community resources; providing ongoing support to individuals while participating in the Supported Housing program and building and maintaining professional relationships with landlords and community partners.
The Homeless Prevention Specialist provides outreach services to individuals exiting state operated prisons, mental health facilities and the foster care system. The goal of this position is to prevent participants from becoming homeless upon their discharge.
- Homeless Prevention Project Specialist services will include; but may not be limited to:
- Outreaching to individuals in state operated prisons/jails, mental health facilities and foster care
- Collaboration with providers at the agencies currently working with individuals
- Entering intake data into HMIS And updating information in HMIS
- Linking with community resources
- Housing search and placement
- Coordinating with landlords/property management companies for participants to obtain housing
- Mediating initial issues with landlords and tenants
- Linking individuals with additional treatment and wraparound services to have ongoing support once participation in the HPP has ended
- Attend weekly team meetings
- Completes all documentation as required by the agency and program.
- Performs other related work as required and assigned.
Qualifications:
- Minimum of a Bachelor’s degree required; Preferably in Psychology, Social Work, Sociology or Human Services.
- Previous Case Management experience preferred.
- Valid Kentucky Driver’s license, insurance and reliable transportation
- Genuine interest and experience in working with individuals who have been diagnosed with an SMI and are at risk for homelessness upon discharge from their current placement.
