Crisis Case Worker (52115)

PRIMARY RESPONSIBILITIES:

(1) Provide emergency service program visits, to include in-home visits, crisis placement, and telehealth to clients in order to monitor their overall functioning in their environment. This will include ongoing assessment, observation, and monitoring mental status and medication.

(2) Provide assistance to clients in the utilization of community resources such as housing, budgeting, food stamps, etc.

(3) Provide or arrange transportation, when it is deemed necessary, to appointments for medical, counseling, and other services necessary to keep the client maintained on an outpatient basis.

(4) Provide psychoeducational training to clients to stabilize mental health crisis situation (individually or group).

(5) Complete all required documentation for the Emergency Service/Crisis program including the development of treatment plans.

(6) Educate and assist clients and their families regarding the client's treatment and care.

(7) Provide psychosocial evaluations.

(8) Work as a part of a multi-disciplined team in crisis services with emphasis on providing client with the least restrictive services and/or referrals.

(9) Assist in deescalating client symptomatology in crisis situations through one-to-one intervention and counseling.

(10) Be available to provide coverage in emergency situations after hours as needed.

(11) Promote Emergency Services/Crisis program throughout the region.

(12) Respond to community crisis situations.

(13) Other duties as assigned/required.

MINIMUM QUALIFICATIONS:

Minimum Bachelor's degree in psychology, social work, or human services related field. Experience in the mental health field is preferred.

Must have a valid driver's license, reliable transportation, and provide proof of automobile insurance as required by the State of Kentucky. Must pass a Criminal Record Check and a Sexual Offender Registry check. Must present proof of a recent (less than 1-year-old) TB skin test.
