PRIMARY RESPONSIBILITIES:
This employee will assist in providing team-based intensive case management services to adult clients whom meet criteria to ensure clients receive necessary services to maintain stability of their SMI symptoms and with the goal of avoiding/reducing hospitalizations and preserving their appropriate placement in the community.
Specific tasks include but are not limited to the following:
- Must be able to establish rapport with clients, families, and cooperating entities.
- Provide ongoing needs assessments for clients.
- Participate in developing treatment plans to ensure clarity, measurability, and applicability to the client.
- Provide ongoing assessment and encouragement in regard to clients Activities of Daily Living (ADL’s).
- Become knowledgeable and utilize LOCUS (Level of Care Utilization System) to identifying appropriate level of care.
- Complete all documentation timely and accurately in the format approved by funding sources and regulatory statutes.
- Act as liaison between client and community; building relationships with families and cooperating entities to develop additional resources and coordinate services.
- Maintain a target number of direct client services and/or clients served as designated annually by the ACT Team Leader in compliance with regulatory statutes.
- Must have transportation and be willing to assist clients in meeting their needs by providing outings, transporting to scheduled appointments with various community advocacy agencies, etc.
- Provide on-call crisis interventions and back up according to team’s policy.
- Other duties as required/assigned by Team Leader and/or supervisors.
Minimum of Bachelor's degree in social work or relevant area with one year experience in MH services preferred. Must pass required training within specified timeframe. Must have current driver's license, proof of automobile insurance, reliable transportation, and be able to travel throughout the region. Must pass a legally required Criminal Record Check and Sexual Offender Registry check.
